Choosing your Support Coordination Agency is your decision. To choose DMG, select Decennium Management Group on your Support Coordination Agency Selection Form and submit it to DDD following the instructions on the form. Once you do, here is what happens:

• We build your Individualized Service Plan to guide your supports.

• Once you are happy with it, your coordinator helps put the plan into action.
