DDD Eligibility

Most people begin accessing DDD services at age 21 or older. To receive DDD-funded services, an individual must be found eligible by the Division and must choose or be assigned a New Jersey Support Coordination Agency — which is where DMG comes in.

To be evaluated for eligibility, visit the DDD website to apply. (Link “visit the DDD website” to the official NJ DDD application page.) There is no cost to you for Support Coordination — it is Medicaid-funded. COMPLIANCE — remove the paragraph that lists “Community Based Supports, Individual Supports, Respite, or Community Inclusion.
