How Support Coordination Works

A Support Coordinator helps you make sense of the DDD system and get the most from the services you qualify for. Working with you and your planning team, your coordinator develops your New Jersey Individualized Service Plan (NJISP), connects you to providers and community resources, monitors how your services are working, and advocates for your goals. Coordinators check in with you regularly — at least once a month — to make sure your plan still fits your life.

Day programs and community inclusion; education and employment resources; in-home supports and personal care; transportation providers; recreation and social activities; respite for caregivers; housing and residential resources; and other DDD-funded services.
